Much of the comedy that was shown on Scrubs was presented in the form of the daydreams J.D. indulged in. Right from the start, he had a habit of tilting his head sideways, looking up, and then losing himself in a fantasy of a situation he had in mind.

These became so frequent over the years that fans now have a certain bunch of them that they consider their favorite fantasies. However, there hasn’t been an official compilation of these fantasies up until now, so we’ve come up with the 10 best ones J.D. thought of over the years. 10 Exploratory Surgery

Since he was a doctor of medicine, J.D. didn’t know the workings behind being a surgeon. So, when Turk told him he would be doing exploratory surgery on a patient to find out what was wrong with him, J.D. took it as Turk being Indiana Jones.

Imagining Turk going literally inside the patient, he envisioned his friend running through traps and trials and happening upon a glowing rock in the colon. The same way Indiana did in his movie, Turk would replace his prize with a decoy and escape the patient to complete his mission. If only surgery was that adventurous in real life, right?

9 Floating Head Doctor

With one of his wishes being that he could be in two places at once, J.D. fantasized about becoming the Floating Head Doctor. This had him literally be a floating head while his body carried out menial chores, although very poorly.

He would imagine simultaneously doing stuff like karaoke singing with his girlfriend while his body prepared dinner, or even getting into a fight where his head battled his own body. The fantasies were very silly, but what can you expect from something that requires a head floating on its own?

8 Star Wars Fantasy

There’s a large, extended world out there where Star Wars is concerned, but even die-hard fans of that series won’t find the version J.D. came up with. When he saw that Dr. Kelso and Cox were fighting over him, he thought himself out to be something like Luke Skywalker.

Imagining the two senior doctors as Obi-Wan and Darth Vader, he saw Cox give away his life (in reality getting suspended) so that J.D. could learn from the event. So deep was J.D. in these thoughts that he even belted out a scream the same Luke did when Obi-Wan had died.

7 Risky Business Fantasy

Every Scrubs fan knows that J.D. pined hard for Dr. Cox’s approval throughout the show’s tenure, even coming up with scenarios where he and Cox would bond. In this instance, he decided to imagine their potential friendship movie-style.

When he imagined what Cox would do in his free time at home, J.D. figured it looked like the underwear dance Tom Cruise had in Risky Business. Except, here he included himself, Turk, and even Todd in the fantasy joining a bemused Dr. Cox.

6 Turk’s Breakdown

As soon as he laid eyes on Elliot’s friend Melody, J.D. knew he was attracted to her. So, when Turk encouraged him to ask her out, J.D. made the conversation even more elaborate than it was. Imagining Turk as a game show host of sorts, he had Melody placed in his mind as something of a sports car in line for a test run.

Full of special effects that come with a game show, Turk was placed as the host who spelled out Melody’s fine points, and more importantly, how she wore no wedding ring. It was perhaps this showmanship he concocted for himself that J.D. found the idea of Melody more alluring.

5 Sacred Heart Who-Caresies Awards

This started out as another one of Dr. Cox’s many disses at the expense of J.D, but inwardly the latter knew he wanted the win. When Cox made up a fake award ceremony for things he cared least about, J.D. compared it to the Emmys.

He even had everyone placed in a situation mere moments before the winner was announced, and was ecstatic at the idea of actually winning, even dissing Tony Shalhoub in his mind as he saw his win even greater than an Emmy.

4 Amigoville

Turk wasn’t sure what to do with his new skill of learning Spanish, as he wasn’t able to find a way to surprise Carla that he knew how to speak her language. To this, J.D. suggested they go and attempt to be friends with laborers by the street.

In his mind, this would unlock the magical land of Amigoville, a paradise for Spanish people where they hang around in parks and eat food too large to fit in one’s mouth. While Turk got away with it due to speaking the language, J.D. imagined himself being kicked out for not fitting in, yet still loved the idea of visiting Amigoville.

3 Surgeon And A Doc Above It All

In the earlier days the doctors of medicine were shown clashing with the surgeons at work, which led to J.D. fantasizing them being part of two warring musical gangs. Since he and Turk loved each other, he saw them having somewhat of a forbidden romance.

To this end, the two had a romantic encounter by the fire escape of their building where they admitted their love for one another despite what others might think. For J.D, they were a “surgeon and a doc above it all.”

2 Sitcom Fantasy

This wasn’t just one skit but a whole episode’s worth of fantasy. Here, J.D. was feeling done with all the pressures of his daily life and wanted things to be as simple as they were on multi-cam sitcoms. So, he began fantasizing Sacred Heart as a comedy show.

Within this setting, things would always work out for the better despite there being impossible odds. Things would also take a comedic route at every turn and J.D. would make miraculous saves while making jokes. It’s too bad the episode’s end snapped this fantasy as we quite liked Scrubs as a multi-cam show.

1 The Future

This is the only fantasy on this list that isn’t meant to be comedic, but it easily had the most heart. Once J.D. had completed all his character development and was looking forward to a big change in his life, he envisioned the perfect future.

This had his son marrying Turk’s daughter, him marrying Elliot, all of his close friends enjoying the holidays, and an altogether idyllic life in store. While the rest of his fantasies had been impossible dreams, this was one dream that J.D. could surely work on making a reality.
