Tormented Souls offers a dark setting, emotional story, and plenty of scares. The game follows Caroline, a young woman that receives a mysterious letter and travels to a gritty hospital for answers. From there, players find clues that shed some light on Caroline’s past. The game’s endings revolve around the fate of Caroline’s twin sister, Anna.

There’s a mix of deadly combat and challenging puzzles that makes Tormented Souls reminiscent of classic horror survival games. Reaching the end of the game involves exploring, surviving, and unlocking the secrets of Wildberger Hospital. Players will have to pay close attention and gather certain items in order to give Caroline and Anna their happy endings.

Like many games, Tormented Souls has secret tasks that unlock different endings. There are three ways to finish the game. In the worst ending, Remorse, Caroline doesn’t escape the mansion. Forgetfulness is the neutral ending, where Anna dies and Caroline escapes. The best possible ending is Adoption: Caroline saves Anna and they escape the hospital together. Each of these endings can be obtained if the player completes (or ignores) specific tasks before attempting to finish the game.

Every Ending in Tormented Souls Explained

The best ending is also the most difficult one to get. The first step is to obtain the antidote (a syringe filled with blue liquid). It can be found in the top left corner of William’s Office after defeating enemies and completing the puzzle to pass through the doors. Later in the game, Caroline will find Noah, defeat the enemies he unleashes, and have the option to give Anna the antidote. Anna will transform back to her normal self, but she won’t survive. On the wall where she was, there’s a set of bolt cutters to pick up. To save Anna, players must return to the VHS projector room after beating the final boss. Put in the blue tape and travel back in time. Use the bolt cutters to cut the chain on the cell where young Anna is being held. Caroline will bring her back to the present, and then they can escape the hospital together.

The neutral ending is similar to the good ending. Players will need to either have the antidote or a weapon to use on Anna. After beating Noah’s monsters, approach Anna. The antidote will cause her to transform, or Caroline can use a weapon to mercy-kill her. Doing either and leaving the hospital directly after beating the boss will result in a neutral ending. Caroline will escape knowing she couldn’t save her sister.

The worst ending is the easiest to get. When the player reaches Anna, they shouldn’t interact with her at all. Continue through the end of the game normally, and use the cross-key to try to escape. A jump scare cutscene will trigger, and Caroline will be attacked and killed by Anna because she was never cured. Tormented Souls has three achievements or trophies related to the endings. Nostalgia is awarded for completing the game regardless of the ending. Players can also get Sisters for the best ending, and Remorse for the neutral ending. All three endings in Tormented Souls revolve around Caroline’s decision regarding Anna’s fate.
